The Massachusetts Historical Society copy has a ms. note at the bottom of the second column: By Capt. Wm. Chandler of Thompson in Connecticut. Printed at Rhode Island. Mass. Historical Society and R.I. Historical Society hold only two known original imprints. Newsbank Readex Early American Imprints series uses RIHS copy.
